Key Responsibilities for Case Management Supervisor:
- Oversee a team of case managers and social workers to ensure effective coordination of patient care and services
- Develop and implement case management processes to optimize patient outcomes and resource utilization
- Collaborate with healthcare providers, insurance companies, and community resources to facilitate seamless transitions of care
- Conduct regular case reviews and provide guidance to staff on complex patient cases
- Monitor and analyze data to track outcomes, identify trends, and implement quality improvement initiatives
- Ensure compliance with regulatory requirements and adherence to best practices in case management
Required Qualifications:
- Bachelor's degree in Nursing, Social Work, Healthcare Administration, or related field
- Minimum of 5 years of experience in case management, with at least 2 years in a supervisory role
- Strong knowledge of healthcare delivery systems, care coordination, and utilization management
- Excellent communication, leadership, and organizational skills
- Ability to work effectively in a fast-paced, multidisciplinary team environment
- Current RN or social work licensure (if applicable)
Desired Skills:
- Master's degree in Nursing, Social Work, or Healthcare Administration
- Certification in Case Management (CCM) or related field
- Experience with electronic health record systems and data analytics
- Knowledge of Medicare and Medicaid regulations
- Bilingual proficiency in Spanish or other languages commonly spoken in the community
Compensation & Benefits:
The annual salary range for the Case Management Supervisor position at Mayo Clinic in Philadelphia, Pennsylvania is $80,000 - $100,000, commensurate with experience.
