Key Responsibilities for Social Worker - Conduct comprehensive psychosocial assessments of patients and families across acute, rehabilitation and community settings, identifying needs related to mental health, housing, finances, and social support. - Develop and implement individualized care plans in partnership with interdisciplinary teams, ensuring alignment with clinical goals and patient preferences. - Provide direct counseling, crisis intervention and therapeutic support to individuals, couples and groups, utilizing evidence‑based modalities. - Coordinate referrals to community resources, government programs and specialty services, and monitor follow‑up to ensure continuity of care. - Advocate on behalf of patients and families to navigate the health system, addressing barriers to access and promoting patient rights. - Document all interventions, assessments and outcomes accurately in the electronic health record in compliance with privacy legislation and Sunnybrook policies. - Participate in multidisciplinary case conferences, discharge planning meetings and quality improvement initiatives. - Contribute to staff education and mentorship programs, sharing best practices in social work and trauma‑informed care. - Maintain up‑to‑date knowledge of provincial regulations, funding streams and emerging trends in health‑social services. Required Qualifications - Bachelor of Social Work (BSW) from a recognized university; a Master of Social Work (MSW) is strongly preferred. - Registration with the Ontario College of Social Workers (OCSW) in good standing. - Minimum of three (3) years of clinical social work experience in a hospital or acute care environment, preferably within a large academic health centre. - Demonstrated competence in assessment, case management, counseling and crisis intervention. - Knowledge of Ontario health legislation, the Ontario Health Insurance Plan (OHIP) and community service delivery models. - Excellent written and verbal communication skills, with the ability to produce clear, concise documentation. - Ability to work collaboratively within multidisciplinary teams and to manage a diverse caseload effectively. - Current CPR and First Aid certification or willingness to obtain within the first 90 days of employment. Desired Skills - Experience with culturally diverse and Indigenous populations, and an understanding of anti‑racist and equity‑focused practice. - Proficiency in motivational interviewing, solution‑focused therapy, and trauma‑informed approaches. - Familiarity with electronic health record systems (e.g., Epic, Cerner) and data reporting tools. - Strong organizational and time‑management abilities, with a track record of meeting deadlines in a fast‑paced environment. - Ability to facilitate group therapy sessions and community workshops. - Bilingual proficiency in English and French is an asset.
